III. Question Types

Choice 

This group of question types allows a fixed single or multiple choice response.


The following options are available:

Question

The title of the question (required)

Additional notes

A guide text to help answer the question

Choice Display

Choose how you display your choices from the following options:
  1. Individual Checkboxes
    1. Shows all responses below the question text,
    2. Useful when there is a small number of choices available
  2. Combined Dropdown
    1. The responses are collected together in a clickable drop down
    2. Better suited for questions with a large number of responses,
    3. Also suits users with small screens and need to save space
  3. Icons 
    1. Choose from a selection of icons to display as responses in place of the text
    2.  Text is stored in the results
  4. Images
    1. Upload an image and display as response in place of the text.
    2. Text is stored in the results

Optional Question

Decide whether a response is required for this question, by default this is set to 'On' (ie required)


Available Choice Types

These are the currently available question types that can be chosen, that preformat the choice question with options to save time:


Yes / No

A simple Yes/No choice Question, displayed as an icon button. The icons and answer text can be altered

Multiple Choice

A Question that has a list of choices, of which the respondent selects one of those choices.

Multiple Choice Grid

A list of Questions that all share the same choices for answering. The respondent selects one of those choices for each Question.



Multiple Choice Dropdown

A Question that has a list of choices collected in a dropdown. The respondent selects one of those choices.

A list of Questions that all share the same choices for answering. The respondent selects one of those choices for each Question.

Checkbox

A Question that has a list of choices, of which the respondent selects one or more of those choices.

Checkbox Grid

Checkbox Dropdown

A Question that has a list of choices collected in a dropdown. The respondent selects one or more of those choices.

Checkbox Dropdown Grid

A list of Questions that all share the same choices for answering collected as a dropdown. The respondent selects one or more of those choices for each Question.

Text

Single Line Text

Allows free text response, field is limited to one line of text, suited for small amounts of text

Multi Line Text


Allows free text response, field is not limited to one line of text, and supports carriage returns. Suited for holding large amounts of text

Email

Checks for the correct format of an e-mail (nb it doesnt check that the email exists)

Telephone Number


Allows the respondent to enter a Phone Number.
A specific Country Phone Number can be selected.

Please Note: This Question will only check the Phone Number is in a valid format, it cannot check that the Phone Number entered actually exists.

Postal/Zip Code


Allows the respondent to enter a Postal/ZIP Code from a specific Country.
Please Note: This Question will only check the Postal/ZIP Code is in a valid format, it cannot check that the Postal/ZIP Code entered actually exists.

Contact Form


This will add 5 Single-Line Text Questions and 1 Postal/Zip Code Question to your form, for use as a contact form for getting name and address from the respondent.




Number

Number Input


Allows a Number to be inputted, between a minimum and maximum value.

Slider

Allows a number to be selected between a minimum and maximum using a slider for convenience

Range

Allows a range of two numbers to be selected between a minimum and maximum using a slider for convenience

Date / Time

Date

A date must be selected from a control. Can automatically be set as the current Date, stored in the format YYYY-MM-DD

Date & Time


A date & time must be selected from a control. Can automatically be set as the current Date, stored in the format YYYY-MM-DD HH:MM



Time


A time must be selected from a control. Can automatically be set as the current Date, stored in the format HH:MM









Stopwatch


Time how long it takes for an action to be achieved. Time can be started, stopped and reset.
Stored in HH:MM:SS:SSS

Rating

Likert Response


A graphical method to find out how much the respondent agrees with a particular statement, using an emotion scale from Strongly Disagree to Strongly Agree.

Star Rating


Used to score something using stars.
By default it is out of 5, but you can add or remove Stars as required.

Other

Location


Get current geolocation when press the Get Location button.

The response will be in the following format with a semi-colon separating the fields: Latitude ; Longitude ; Accuracy (Metres).


The view data section of the application show this as a mini-map



Clicking on the mini map, shows a bigger map


Scan Barcode

This Question allows the respondent to scan a barcode via their device camera, and store the value of that barcode.

A list of supported Barcode types are shown below:



Please Note: Website Users can only scan using a Built-In / USB Camera.


If the scanner isnt functioning correctly (usually due to the camera) the facility to manually enter the code is

Formatting

Label

A formatted piece of information that can be used to guide the user through the survey
